Prepare for First Visit

Preparing for Your Oral and Maxillofacial Surgery Consultation at The Pogue Center in Scottsdale, AZ

Step-by-Step Guide to Your First Oral and Maxillofacial Surgery Appointment – the Consultation

Patient Registration & Health History

Your visit begins with completing new patient forms. It’s best to complete and submit these one week prior to your consultation so our team has time to review and verify insurance, so we can be prepared to discuss insurance coverage and costs.

On the Day of Your Consultation

  • Bring your valid Photo ID
  • Bring your referral form if one is provided to you by your referring physician
  • Bring your valid insurance cards (Medical and Dental)
  • Bring an updated list of current medications (if what you provided on your new patient paperwork was incomplete or if anything may have changed)
  • Be prepared to discuss your medications, medical history, previous surgeries, etc.

During Your Consultation, the Oral and Maxillofacial Surgeon Will...

  • Review your health records and referral notes
  • Examine and evaluate your mouth, jaw, or affected area
  • Discuss symptoms or concerns
  • Take or review dental x-rays or 3D imaging

Personalized Treatment Planning

Based on your evaluation, the surgeon will create a customized treatment plan. This includes:

  • Recommended procedures
  • Anesthesia / Sedation
  • Risks, benefits, and recovery expectations
  • Pre-op and post-op instructions

Scheduling Your Procedure

After your treatment plan is finalized, you may schedule your surgery.
