Usual and Customary

Each insurance company determines what they consider to be the usual and customary charge for a specific procedure/visit. These determinations vary between insurance companies and no two are ever the same. If our fee is over their determined amount, the patient is expected to pay the difference between the two. The patient is permitted to appeal the decision to their insurance provider; however, any unpaid amounts become the patient's responsibility to pay
